What is PCOS/ PCOD?

PCOS is the most common endocrine disorder in the women of reproductive age. It is a lifestyle disorder. PCOS as the name suggests is Polycystic Ovarian Syndrome and PCOD means Polycystic Ovary Disorder both are similar. Here ovaries instead of making one single mature ova produces many cysts. It is more commonly seen in obese girls or women who lead a sedentary lifestyle and are not into regular active exercise or possess a family history of diabetes and PCOS.

Symptoms and signs of PCOD –

  1. Irregular Menstrual Periods or Missed Periods i.e. having no periods for 1 or 2 months or sometimes excessive menstrual flow.
  2. Acne – Acne or pimples that do not improve despite skin treatments.
  3. Excessive Facial Hair Growth – Thick hair growth in areas where females generally don’t have hair.

Pathophysiology

Exact pathophysiology of PCOS is not clearly understood.

Clinical Features

The patient complains of –

  1. Over Weight or obesity (abdominal 50%),
  2. Menstrual irregularity (70%) – inconsistent menstrual blood flow (oligomenorrhea- less than 9 cycles per year), absence of menses (amenorrhea) or dysfunctional uterine bleeding (DUB) and infertility.
  3. Hyperandrogenism – Presence of Acne (acne vulgaris), hirsutism (excessive hair growth) and hair loss or thinning of hair are the characteristic features. (70%)
  4. Acanthosis nigricans is characterized by specific skin changes because of insulin resistance. In this, skin is thickened, dark-velvety and pigmented (grey brown). Commonly affected sites are nape of neck, inner thighs, groin and axilla.
  5. HAIR-AN syndrome in patients with PCOS is characterized by hyperandrogenism, insulin resistance and acanthosis nigricans.

Causes of PCOS/PCOD

PCOS is caused by genetic and environmental factors. Genetic factors include family history of diabetes or family history of PCOS. In families with higher number of diabetes patients or where mothers, grandmothers, aunts themselves have had PCOS or difficulty in conceiving; in these cases PCOS is more commonly observed.

Poor lifestyle is the main environmental factor. In today’s lifestyle many girls and students mostly lead a sedentary lifestyle, sitting for long hours either due to studies or work. There is a lack of walking, exercise or maintaining regular fitness. Intake of sugary drinks, juices, cold drinks, pizza, pasta, burgers, fast food and the consumption of alcohol and smoking are all responsible for the increased incidence of PCOS nowadays.

This heterogeneous disorder is primarily marked by excessive androgen production from the ovaries. PCOS is considered both multifactorial and polygenic. Research has indicated possible dysregulation of the CYP 11a gene and upregulation of enzymes involved in androgen synthesis. Furthermore, the insulin receptor gene on chromosome 19p 13.2 is also implicated in the condition.

PCOS often runs in families and suggesting a genetic component. Researchers have identified several physiological factors that could increase the risk of developing PCOS including – 

  • Excess Insulin Production – Insulin is a natural hormone produced by the pancreas that plays a main role in regulating the body’s metabolic functions of body and regulate blood sugar levels. High insulin levels are considered a major factor in the development of PCOS. Increased insulin also can lead body to produce more androgens (male sex hormone) which can disrupt ovulation.
  • Inflammation – Various physiological factors can lead to low-grade inflammation within the body. Mild autoimmune disorders can also triggers inflammation in bodily tissues which in turn can increase androgen levels.
  • High Androgen levels– Androgen also called male sex hormones. They  are linked to several symptoms when present in high levels in females. These include increased facial and body hair, acne outbreaks and skin issues.

Investigations

  • Ultrasonography: Transvaginal ultrasound is especially useful in obese patient. Ovaries are enlarged in volume (>10 cm3). Increased number (>12) of peripherally arranged cysts (2-9 mm) are seen.
  • Blood tests can be used to identify characteristic changes in hormone levels although these changes are not universal. Women with PCOS may have Increased levels of:
    • Testosterone – It is an ovarian androgen hormone that affects hair growth;
    • Luteinising hormone (LH)- It is a pituitary hormone that plays an important role in ovarian hormone production & is essential for ovulation;
    • FSH (Follicular Stimulating Hormone) – It is a pituitary hormone that plays a crucial role in ovarian hormone function. It stimulates follicles on ovary to grow and prepare eggs for ovulation;
    • Oestrogen – an ovarian hormone that promotes growth of the endometrium (lining of womb); and
    • Anti-müllerian hormone (AMH) – It is an indicator of the ovaries fertility potential.
    • Ratio of LH:FSH is > 2:1.
    • SHBG (sex hormone binding globulin) level is typically lowered.

Diet for PCOD / PCOS

  1. Diet rich in fruits and vegetables.
  2. Minimizing dairy products as much as possible.
  3. Adequate hydration by consuming at least 2-3 litres water daily along with other options like coconut water, buttermilk, vegetable soups.
  4. Put a stop to the carbonated, fizzy and sugary beverages.
  5. Avoid saturated and hydrogenated fats but choose unsaturated fats.
  6. Select whole wheat products such as wheat pasta, wheat poha &
