A girl in her early 20s came to us with severe pain and bleeding while passing stool. Her complaint began a few months ago when she started to experience difficulties with her bowel movements. As her stools were very hard for the past 1year. In spite of receiving regular treatment from a reputed hospital in Delhi. Family history - Her grandmother had diabetes, ankylosing spondylitis, and hypertension. Her mother had a positive rheumatoid factor and her maternal grandmother also had diabetes.
Despite her bowel issues, she had a normal appetite. She drank about 4-5 glasses of cold water each day (thirstless), and her urine output was normal. She also suffered from dryness of her lip.
